10 REQUIREMENT FOR NATIONAL IDENTIFICATION NUMBER MODIFICATIONS

To modify National Identification Number data, individuals must submit supporting documents and complete the required forms at a designated National Identity Management Commission office.
The specific requirements may vary depending on the type of modification.
The general requirements, however, are:
- Completed NIN modification form (obtained from the NIMC office or downloaded from the NIMC website)
- Original NIN slip (for verification purposes)
- Supporting document(s) for the modification (e.g., birth certificate, marriage certificate, court order, etc.)
- Proof of identity (any of: passport, driver’s license, permanent voter’s card, etc.)
- Proof of citizenship or residency (any of: birth certificate, passport, naturalization certificate, etc.)
- Recent passport-sized photograph
- Fingerprints (captured at the NIMC office)
- Signature (signed on the modification form)
- Payment of the applicable modification fee (if required)
- Completed and signed consent form (for minors or individuals with legal guardians)
